Some foreskins are loose and some are tight. However, no foreskin should be so tight that it hurts to try to retract it. If this is the case, it's best that you discuss the condition with a doctor. There is a simple procedure, which can be done right in the office, that involves making a small incision in the foreskin, with a Novacaine-like anesthetic. (It's not nearly as extreme as a circumcision.) Moderately tight foreskins that are not quite as tight can be loosened slightly by daily gentle stretching and massage exercises with a lotion or petroleum jelly; unfortunately we don't have any specifics on how to do this.
